LOSS CONTROL Planning CHECKLIST
The checklist is divided into 6 sections,
each section address a specific aspect of the event
Section 1 - Event Information
This section covers specific event information
including date, time, location etc.
Section 2 - Site Selection This
section covers issues related location
identified in section 1
Section 3 - Food & Refreshments
This section covers safety issues related to service of food or
drinks including alcoholic beverages.
Section 4 - Operations This
section covers issues related operational plans including
mandatory Public Safety notification and emergency
planning
Section 5 - Activities Involving Fire,
Flame and/or Heat This section covers issues
related to activities that involve any fire or flames such as fireworks
display, barbeque, etc.
Section 6 - Insurance & Contracts
This section covers issues related event liability evaluation, contracts and waivers.
Sections 1, 2, 4 and 6 are
mandatory
Section 3 must be completed if refreshments will be served
or sold
Section 5 must be completed if any activity involves fire or
heat
Once you have completed the checklist print it out and faxt it to 305-348-3574 or deliver it to room CSC 162 Univ. Park to submit your request.
