         Any document requiring student conduct verification can be

         hand delivered, mailed or faxed to Student Conduct & Conflict 

         Resolution (Graham Center; room 155)

                        Please make sure you have:

 

 

                               ●Signed and dated your certification (s)

                               ●Enclosed a stamped self-addressed envelope for

                                            each certification

                               ●If you chose not to provide a stamped self-   

                                            addressed envelope you may:

                                              Provide us with your telephone number for us to

                                                 call when the form is ready to be picked up.

                                              If the institution permits, a fax number for

                                                 our office to fax directly to the institute.

                               ●The Registrar's office in PC 130 is the only

                                            office that may verify date of enrollment and 

                                            rank in class. (305) 348-2320

                                                 

             The Student Conduct & Conflict Resolution office will verify academic 

                 good standing with the Academic Affairs office. Once the document (s)

                 have been completed with the judicial and academic standing

                 information, we will mail the document to the requested school using

                 your self-addressed envelope or fax the document (s) to the school.

                 This process takes approximately 7-10 business days. During the

                 holidays and application deadlines the process takes 10-14 business
